Please take a moment to review, download and print the forms and documents available. The documents describe in greater detail the services provided by STAT EMS; the forms listed may have to be completed and submitted to qualify for insurance coverage for ambulance services (please note, forms may change depending on insurance carrier). We recommend each patient download and review all literature regarding medical transportation.

STAT EMS’s Patient Business Services manages all patient billing services through an internal billing department. Our billing policies strictly adhere to state and federal reimbursement regulations. In addition, our policies also educate our personnel as well as hospital and nursing facility personnel about issues of compliance and documentation.
